While frequently employed as an antimicrobial agent in both human and veterinary applications, sulfadiazine's presence in the environment, however small, poses a significant concern as an emergency pollutant. A rapid, discerning, sensitive, stable, reversible, reproducible, and user-friendly monitoring system is crucial. A carbon-modified electrode, coupled with electrochemical techniques like cyclic voltammetry (CV), differential pulse voltammetry (DPV), and square wave voltammetry (SWV), stands as an excellent, cost-effective and convenient analytical approach, streamlining the control process while also guaranteeing the protection of human health from potential drug residue buildup. The development of the academic field of prosthetics and orthotics (P&O) over recent years has corresponded with a significant rise in scientific research in this area. Yet, the quality of published studies, particularly those categorized as randomized controlled trials, is not always deemed acceptable. Subsequently, this research project intended to evaluate the methodological and reporting rigor of randomized controlled trials within the Iranian Perinatal and Obstetrics field, in order to recognize areas for potential enhancement.
In the period from January 1, 2000, to July 15, 2022, six electronic databases (PubMed, Scopus, Embase, Web of Science, the Cochrane Central Register of Controlled Trials, and the Physiotherapy Evidence Database) were searched comprehensively. The methodological standards of the studies that were included in the review were evaluated using the Cochrane risk of bias tool. The Consolidated Standards of Reporting Trials (CONSORT) 2010 checklist was applied to assess the reporting quality of the studies that were part of the review.
After thorough review, we chose 35 randomized controlled trials (RCTs), published between 2007 and 2021, for our final analysis. Poor methodological quality characterized 18 RCTs, while a group of 7 studies exhibited high methodological quality, and 10 studies showed a moderate degree of methodological quality. The middle value (interquartile range) of reporting quality for RCTs, in line with the CONSORT criteria, was 18 (13–245) out of a total of 35. The relationship analysis indicated a moderately correlated trend between the CONSORT score and the year of publication for the RCTs included in the research. Despite this, a weak relationship existed between CONSORT scores and the impact factors of the journals.
The P&O RCTs conducted in Iran exhibited a methodological and reporting quality that was suboptimal. To refine the methodology's quality, stricter attention should be paid to aspects such as masking of outcome assessment, concealed allocation, and the generation of random sequences. This research aims to evaluate the presence of TORCH infections in a child with bilateral cataracts and hearing loss, and report the ToRCH serological profile (Toxoplasma gondii [TOX], rubella [RV], cytomegalovirus [CMV], and herpes simplex virus [HSV I/II]) within the pediatric population presenting with both cataracts and deafness.
Cases in the study possessed a clinically evident history of congenital cataracts and congenital deafness. Eighteen cases of bilateral cataracts and twelve instances of bilateral deafness in children were admitted to AIIMS Bhubaneswar for, respectively, cataract surgery and cochlear implantation. A sequential analysis of IgG/IgM antibodies against TORCH agents was performed qualitatively and quantitatively on sera collected from all children.
Every patient suffering from cataract and deafness had antibodies against the torch panel, specifically anti-IgG antibodies. Analysis of bilateral cataract children revealed anti-CMV IgG in 17 of 18 cases, consistent with the findings in 11 of 12 bilateral deaf children. Anti-CMV IgG antibody positivity rates demonstrated a statistically significant increase. Anti-CMV IgG was detected in 94.44% of cataract patients and 91.66% of those with hearing loss. In addition, a significant proportion of patients, 777% from the cataract group and 75% from the deafness group, displayed the presence of anti-RV IgG antibodies. Bilateral cataract patients with positive IgGalone antibodies were primarily linked to Cytomegalovirus (94.44%, 17/18 cases). The next most frequent pathogen was Rhinovirus (77.78%, 14/18 cases), followed distantly by Human Herpes Virus 1 (HSV1) (27.78%, 5/18), Toxoplasma (TOX) (27.78%, 5/18), and Human Herpes Virus 2 (HSV2) (16.67%, 3/18). For patients experiencing bilateral deafness, the distribution of IgG-alone seropositive cases was almost consistent, save for the absence of TOX (zero out of twelve).
In pediatric cases of cataracts and deafness, the current study highlights the importance of cautious interpretation of ToRCH screening data. To ensure accurate diagnosis, serial qualitative and quantitative assays must be combined with clinical correlation within the interpretation process, minimizing potential errors. Sero-clinical positivity testing is required for older children, who might contribute to the spread of the infection.
The current study recommends that clinicians exercise caution when interpreting ToRCH screening results in children presenting with both cataracts and deafness.
